Service 1

Wound Evaluation

For wounds that don’t need to be followed by a wound specialist, appropriate wound evaluation and management will be given to the treatment nurse.

In-House Patients with New Wounds

Wounds will be appropriately diagnosed and discussed with the treatment nurse on ways to aggressively close the wound within 30-60 days if achievable.

Service 2

Emergency G-Tube Replacement

Dislodged or clogged G-tubes that need urgent replacement will be replaced within 2-12 hours.

For longer wait periods, nurses will be advised on how to hydrate patients, especially diabetic patients.

Nurses will be kept up to speed on when the G-tube will be changed, stat KUB to be ordered and contrast left with them.

Service 3

Biopsies

Biopsies will be performed on patients that need urgent consult to ensure proper collaboration with necessary entities like Rheumatology, Dermatology, or Oncology.

This reduces time spent on consulting with PCP, waiting for a dermatology consult, and therefore delaying proper rash/ulcer management.

Example: A patient had an irregular hyperpigmented area that was increasing in size. A biopsy was performed, and within two weeks, the patient was able to know that it was melanoma. The patient had an oncology appointment and MOH excision done within two weeks of diagnosis. This particular patient was extremely happy with the services provided.

Service 4

The attending Physician for San Diego Wound Experts, Michael Griesinger, MD.

He attended Cornell Medical College with Emergency Room training at Kings County Hospital in Brooklyn, NYC, and completed a fellowship at Kaiser Permanente with a focus on Ultrasonography.

The attending physician can be called on for wound patients if needed to evaluate and manage patients, especially when unsure whether to send the patient to the ER or manage them in-house. This helps reduce unnecessary ER visits for patients at the SNF.
